Care Manager (Behavioral Health)

Location: Remote; applicants must reside in Kansas. Schedule: Monday through Friday, 8:00 AM to 5:00 PM.

Salary: $56,200–$101,000 per year.

What you will do

• Evaluate member needs through phone, digital, home, and other site visits, including behavioral health and social determinant needs.
• Develop and monitor personalized care plans for members with high-acuity mental health and substance use disorder needs.
• Coordinate services among members, families, caregivers, providers, specialists, community resources, and multidisciplinary teams.
• Monitor treatment outcomes, symptoms, complications, and side effects, recommending updates to care plans when needed.
• Educate members and families about treatment options, referrals, provider instructions, procedures, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost effectiveness.

Requirements

• Must reside in Kansas while working remotely.
• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• Active Licensed Master’s Behavioral Health Professional license required, such as LCSW, LMSW, LMFT, LMHC, or LPC; alternatively, an RN qualification with behavioral health experience may meet state contract requirements.
• Two to four years of related experience.
• Strong behavioral health clinical knowledge, assessment skills, communication, and customer service skills.
• Experience with discharge planning, care coordination, problem-solving, documentation, time management, and multidisciplinary collaboration is highly preferred.
