Care Manager (RN)

Location: Remote in Ohio; candidates must reside within designated Ohio counties and perform local travel and in-person member visits.

Salary: $56,200–$101,000 per year.

What you will do

• Assess member healthcare needs, access barriers, and social determinants of health.
• Develop and update personalized care plans and service plans with members, families, caregivers, providers, and specialists.
• Coordinate timely access to healthcare, community resources, referrals, and other support services.
• Monitor member status, changes in condition, progress toward goals, and care plan effectiveness.
• Provide education on disease processes, care gaps, provider instructions, treatment options, referrals, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• 2–4 years of related experience.
• RN state licensure and/or compact state licensure.
• Must be authorized to work in the United States without current or future employment-based visa sponsorship.
• Must reside in designated Ohio counties and be able to perform local travel and in-person member visits.
