Onsite Care Coordination Nurse

Location: Hybrid/onsite at Camden Clark Memorial, 800 Garfield Ave, Parkersburg, West Virginia; must live within a reasonable 30–60 minute driving distance. Limited remote work is available for coordination and documentation. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support discharge planning from admission through post-discharge and coordinate follow-up care with primary care providers.
• Identify care gaps, safety concerns, and barriers to discharge, then implement strategies to address them.
• Collaborate with attending providers, facility case managers, and other healthcare professionals to coordinate care.
• Educate members and families about diagnoses, medications, preventive measures, and ongoing wellness.
• Complete documentation accurately while following infection control, safety, privacy, and HIPAA standards.

Requirements

• Current, unrestricted Registered Nurse license in West Virginia.
• Must be located within a 30–60 minute driving distance of Camden Clark Memorial in Parkersburg, West Virginia.
• At least three years of recent acute care nursing experience, such as medical-surgical, critical care, heart/lung, skilled nursing, or rehabilitation.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, personal vehicle liability insurance, TB screening, background clearance, and a private workspace for remote documentation.

Nice to have

• Bachelor’s degree in Nursing.
• Experience in care management, health promotion, coaching, utilization management, or health plan operations.
• Bilingual English/Spanish communication skills.
• Care management or related specialty certification.
• Knowledge of HEDIS and Stars measures.

• Monday–Friday schedule from 8:00 AM to 5:00 PM local time, with no holidays, weekends, or on-call work.
• 40-hour workweek with a hybrid arrangement combining facility-based member engagement and limited remote documentation.
