Lead Care Manager - High Desert (ECM)

Location: Hybrid in Victorville, California, United States; 2 days remote and 3 days in the field across High Desert regions. Travel within Orange County is required.

Salary: $21.00–$25.00 per hour, plus quarterly bonuses.

What you will do

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify gaps in care and maintain accurate, timely documentation.

Requirements

• Experience in care or case management, preferably in a community-based setting.
• Knowledge of Medi-Cal and Enhanced Care Management is strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.
