LTSS Service Care Manager (Healthcare)

Location: Field-based position; applicants must reside within 30 miles of South Johnson, Miami, and/or Franklin County, Kansas. Regular schedule is Monday through Friday, 8:00 AM to 5:00 PM, with evening and weekend work as needed. Home and other site visits may be required.

Salary: $27.02–$48.55 per hour

What you will do

• Assess long-term care members' needs, available resources, and appropriate care outcomes.
• Develop and monitor personalized long-term care and service plans.
• Coordinate members, families, caregivers, healthcare providers, specialists, and community resources.
• Provide support related to housing, employment, independent living, participant direction, justice, and foster care needs.
• Educate members and caregivers about healthcare instructions, service options, referrals, procedures, and benefits.
• Document care management activities, monitor outcomes, support regulatory compliance, and recommend quality improvements.

Requirements

• Bachelor's degree and 2–4 years of related experience, or equivalent qualifying experience.
• Relevant education may include health, human, social work, or education services.
• Experience with case management for older adults, behavioral health conditions, physical or developmental disabilities, or HIV/AIDS populations may qualify.
• Knowledge of HCBS waivers, behavioral health, community-based referrals, and available resources.
• Strong assessment, communication, customer service, time-management, organizational, problem-solving, and computer skills.
• Must be able to work independently in a community-based environment and reside within the specified Kansas service area.
