Focus on conducting phone outreach and home visits, maintaining continuity of care, and supporting members throughout service delivery in assigned Arkansas counties.

Location: Field-based role serving Phillips, Monroe, and Lee Counties, Arkansas; candidates must reside in an assigned county or an Arkansas border city. Travel: 30%.

Salary: $17.84–$28.02 per hour

What you will do

  • Conduct member outreach by phone and home visits to discuss care plans, services, resources, and next steps.
  • Coordinate care activities with healthcare providers, community organizations, members, caregivers, and internal care teams.
  • Respond to member and provider questions or concerns related to care and service plans.
  • Support service assessments, screenings, referrals, member education, and continuity of care.
  • Document and maintain member records in accordance with state, contractual, and regulatory requirements.

Requirements

  • High school diploma or GED; Arkansas Total Care candidates may alternatively qualify with a bachelor's degree in social science or a health-related field.
  • One to two years of related experience; Arkansas Total Care applicants with a high school diploma need at least one year coordinating care for developmentally or intellectually disabled or behavioral health clients.
  • Must reside in Arkansas or an Arkansas border city and be able to travel approximately 30%.
  • Valid driver's license, child and adult maltreatment checks, and drug screening are required for the Arkansas position.
  • Ability to coordinate care, identify member needs, make referrals, maintain records, and follow regulatory standards.
