Care Coordinator II

Location: Field-based role serving Columbia and Union counties in Arkansas; candidates must reside in an assigned county, Arkansas, or a border city. Travel: 30% required.

Salary: $17.84–$28.02 per hour

What you will do

• Contact members by phone and through home visits to discuss care plans, services, resources, and next steps.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and other partners.
• Support member and provider inquiries and communicate with care managers and practitioners to maintain continuity of care.
• Assist with service assessments and screenings, identify member needs, and make referrals to appropriate care or community resources.
• Document and maintain member records in accordance with state, contractual, and regulatory requirements.
• Educate members about available benefits and resources while following applicable policies and standards.

Requirements

• High school diploma or GED.
• 1–2 years of related experience.
• For the Arkansas Total Care plan: a bachelor's degree in social science or a health-related field, or a high school diploma with at least one year of experience coordinating care for clients with developmental or intellectual disabilities or behavioral health needs.
• Must reside in Arkansas or a border city and be able to travel approximately 30%.
• Arkansas safety-sensitive role requiring a driver's license, recurring child and adult maltreatment checks, and drug screening at hire and on a recurring basis.
