Part-Time MTM Pharmacist - Remote

Location: United States work-from-home; remote position

Salary: USD 54–91 per hour, depending on experience and geographic location; eligible for an annual bonus plan.

What you will do

• Provide Medication Therapy Management services, including Comprehensive Medication Reviews, Targeted Medication Reviews, adherence interventions, and cost-effective clinical interventions.
• Assess medication therapy for safety concerns, drug interactions, therapeutic duplication, gaps in therapy, adherence barriers, and treatment optimization opportunities.
• Educate and counsel patients and communicate clinical recommendations to prescribers and other healthcare professionals.
• Conduct patient and provider outreach to resolve medication-related concerns and support medication management services.
• Provide pharmacogenomic consultations, transitions-of-care support, adherence and quality initiatives, and other assigned clinical programs.
• Document reviews, interventions, recommendations, outreach, and outcomes while collaborating with pharmacy technicians and interdisciplinary teams.

Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
• Active pharmacist license in good standing.
• At least three years of experience in MTM, ambulatory care, population health, telepharmacy, medication adherence, pharmacogenomics, or other patient-centered pharmacy services.
• Strong clinical assessment, communication, organization, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment and collaborate with a multidisciplinary team.
• Must maintain a HIPAA-compliant home workspace and use cable broadband or fiber internet with at least 10 Mbps download and 5 Mbps upload.

Nice to have

• Bilingual Spanish-speaking ability.
• Experience with electronic health records, clinical documentation platforms, pharmacy software, and standard productivity applications.
