Patient Care Advocate (Healthcare)

Location: Remote within the United States; candidates must be located in Maricopa County, Arizona

What you will do

• Act as a liaison and advocate between members, families, physicians, facilities, and community agencies.
• Conduct telephonic outreach, health coaching, and face-to-face education to close preventive care gaps.
• Educate provider practices on HEDIS measures, ICD-10 and CPT coding, and NCQA requirements.
• Analyze provider performance data and collaborate with Provider Relations on quality, risk adjustment, claims, and encounter improvements.
• Schedule appointments, arrange transportation, connect members with community resources, and coordinate follow-up care.
• Document member interactions, maintain confidentiality, resolve issues, and refer members to case or disease management.

Requirements

• Bachelor’s degree in healthcare, public health, nursing, psychology, social work, health administration, or a related health field, or equivalent experience.
• Four years of total experience, including at least two years in direct patient care, social work, quality improvement, or health coaching.
• Experience in a managed care environment is preferred.
• Fluency in English and Spanish is highly preferred.
• Must be authorized to work in the United States without current or future employment-based visa sponsorship.
• Candidates must be located in Maricopa County, Arizona.

Nice to have

• Licensed Practical Nurse, Licensed Master Social Worker, Certified Social Worker, Licensed Social Worker, or Registered Nurse certification.
