Senior Integrated Clinic Pharmacist

Location: Remote, but must reside within a 50-mile radius of Corpus Christi or El Paso, Texas, United States. Schedule: Monday-Friday, 8:00-17:00 Eastern Time. Travel: approximately 5% for market meetings.

Salary: $126,300-$173,700 per year, plus eligibility for a bonus incentive plan.

What you will do

• Evaluate and optimize pharmacotherapy for high-risk patients with diabetes, hypertension, heart failure, COPD, and other chronic conditions.
• Collaborate with physicians, nurses, and multidisciplinary care teams, including participation in high-risk rounds.
• Provide patient counseling, disease-state education, provider consultation, and post-discharge medication reconciliation.
• Optimize formulary coverage and lower-cost medication options while supporting appropriate care.
• Document clinical interventions and patient care activities in the clinic EMR and pharmacy management systems.
• Support CMS Stars, HEDIS, medication-safety, quality-improvement, staff-training, and targeted pharmacy initiatives.

Requirements

• Active, unrestricted pharmacist license in Florida and willingness to obtain additional state licenses for market support.
• Pharm.D. or another advanced pharmacy degree and at least five years of practicing pharmacist experience.
• Experience working collaboratively in a team environment and supporting shared goals.
• Proficiency with Microsoft Office applications, remote-work troubleshooting, and basic technical problem-solving.
• Multilingual ability, preferably English and Spanish, with successful completion of both language assessments.
• Ability to work remotely while residing within the specified radius of Corpus Christi or El Paso and maintaining Eastern Time hours.

Nice to have

• Clinical residency in ambulatory care or a similar discipline.
• Board certification in geriatric pharmacy.
• Ambulatory care or primary practice experience.
• Familiarity with EMR systems and chronic-condition drug therapy management.
• Experience implementing innovative solutions that improve efficiency and patient outcomes.
