Lead Care Manager - Low Desert (ECM)

Location: Hybrid role based in Palm Desert, United States, with 2 days remote and 3 days in the field across Low Desert regions. Areas covered include Palm Desert, Indio, and Palm Springs. The role also requires willingness to travel within Orange County.

Salary: $21.00–$25.00 per hour, plus quarterly bonuses.

What you will do

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and healthcare providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify gaps in care and maintain accurate, timely documentation.

Requirements

• Experience in care management or case management; community-based experience is preferred.
• Knowledge of Medi-Cal and Enhanced Care Management is strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County and work three days per week in the field.
• Bilingual English/Spanish communication skills are preferred.
