Case Manager (Healthcare)

Location: Fort Atkinson, Wisconsin / Jefferson County, Wisconsin. This is a remote field position with services provided at members' homes and other non-company locations, up to 40% travel throughout Jefferson County. Typical hours are Monday–Friday, 8:00 am–4:30 pm CST.

Salary: $53,700–$72,600 per year.

What you will do

• Provide comprehensive, person-centered social service care management for Family Care members.
• Learn each member's goals and support progress toward daily living, wellness, and community participation.
• Partner with a Health and Wellness Coordinator and collaborate on member needs and care plans.
• Conduct enrollment and follow-up face-to-face assessments, quarterly visits, and monthly phone contacts.
• Develop, review, coordinate, and document member plans of care.
• Lead support related to social services, community integration, employment, housing, and other non-health needs.

Requirements

• Bachelor's degree in Human Services or a related field plus one year of experience with a Family Care target population, or a bachelor's degree in another field plus three years of related experience.
• Experience must involve frail elders or adults with intellectual, developmental, or physical disabilities.
• Valid state driver's license and personal vehicle liability insurance with minimum 100/300/100 limits required.
• Ability to travel up to 40% throughout Jefferson County, Wisconsin and perform face-to-face member visits required.
• Ability to work with member protected health information in a dedicated, interruption-free workspace; required internet speeds are at least 25 Mbps download and 10 Mbps upload.
• TB screening is required because this is a patient-facing role.
