Case Manager

Location: Remote, serving members in Menasha, Wisconsin and the Calumet, Outagamie, and Winnebago coverage areas. Travel up to 40% is required, including recurring in-person member visits and occasional travel to hirify.global offices for training or meetings. Working hours are Monday-Friday, 8:00 am-4:30 pm CST.

Salary: $53,700-$72,600 per year.

What you will do

• Assess members' health, safety, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and other services to support members in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, typically at members' residences.
• Complete quarterly in-person visits and maintain monthly phone contact with members.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, educate members, manage service authorizations, and maintain accurate case documentation.

Requirements

• Bachelor's degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor's degree in another field plus at least three years of relevant experience.
• Valid state driver's license and personal vehicle liability insurance with minimum limits of 100/300/100 are required.
• Ability to conduct recurring in-person visits and travel up to 40% within the assigned Wisconsin coverage area.
• Ability to work Monday-Friday, 8:00 am-4:30 pm CST.
• Ability to work from a dedicated home space with internet speeds of at least 25 Mbps download and 10 Mbps upload while protecting member PHI and HIPAA information.
• Successful completion of required TB screening for this patient-facing role.
