Onsite Care Coordination Nurse (Healthcare)

Location: Hybrid/onsite at Atrium Health Wake Forest Baptist Medical Center, 1 Medical Center Blvd, Winston-Salem, North Carolina; candidates must live within a 30–60 minute driving distance and maintain a private workspace for remote documentation.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support discharge planning, educate members and families, and coordinate post-discharge primary care follow-up.
• Identify care gaps, safety concerns, and discharge barriers, then develop strategies to address them.
• Collaborate with providers, facility case managers, and healthcare professionals to coordinate timely care.
• Educate members about diagnoses, medications, and preventive measures to reduce readmission risk.
• Complete documentation while following infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted North Carolina Registered Nurse license required.
• Must live within a 30–60 minute driving distance of the assigned Winston-Salem facility.
• At least 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, personal vehicle liability insurance, TB screening, and required background and facility credentialing clearances.

Nice to have

• Bachelor’s degree in Nursing, care management or related certification, and health promotion or coaching experience.
• Utilization management or health plan experience.
• Bilingual English/Spanish communication skills.
• Knowledge of HEDIS and Stars measures.
