Case Manager (Healthcare)

Location: Remote Flex - North Carolina, United States. Remote work is expected with a few in-office visits each year; candidates may apply from eligible states listed on the posting. hirify.global's headquarters are in Durham.

Salary: $73,698.00–$117,917.00 annually, plus an annual incentive bonus.

What you will do

• Coordinate care, resources, and services for members as part of a multidisciplinary team.
• Conduct comprehensive clinical assessments and develop individualized, member-centered care plans.
• Monitor progress, arrange follow-ups and referrals, provide education, and modify care goals as needed.
• Facilitate care across physician offices, hospitals, rehabilitation units, skilled nursing facilities, and home care.
• Evaluate medical necessity, treatment alternatives, benefits, and available community resources.
• Document assessments, interventions, outcomes, and care-plan changes; develop policies or training materials when needed.

Requirements

• Registered Nurse with at least 3 years of clinical and/or case management experience.
• Valid North Carolina or compact multistate clinical licensure required and must be maintained.
• Additional specialty certification, such as CCM or CDCES, may be required for some roles and must be obtained within two years when applicable.
• Employment authorization sponsorship is not available.
