Care Manager, Telephonic Nurse

Location: Remote in South Carolina; must reside in South Carolina. Occasional travel to hirify.global offices for training or meetings may be required, with up to 20% travel for business needs.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic assessments of members’ clinical, behavioral, social, and environmental needs.
• Develop, implement, and update individualized care plans while monitoring progress toward health outcomes.
• Provide education and support for chronic condition management and self-care.
• Coordinate access to providers, services, and community resources and resolve barriers to treatment adherence.
• Collaborate with members, families, providers, and interdisciplinary teams to support care goals.
• Maintain accurate documentation and independently manage assigned caseloads in accordance with quality and compliance requirements.

Requirements

• Active, unrestricted South Carolina Registered Nurse license with multistate compact privileges and no disciplinary action.
• Must reside in South Carolina.
• At least 3 years of recent acute care clinical nursing experience.
• Experience supporting chronic condition management and coordinating community resources.
• Proficiency with Microsoft Office applications, including Word, Excel, and Outlook.
• Strong documentation, multitasking, computer navigation, and basic troubleshooting skills.

Nice to have

• Bachelor of Science in Nursing and Certified Case Manager certification.
• Experience in telephonic case management, care management, discharge planning, transitions of care, or adult acute care patient education.
• Managed care, Medicare and Medicaid, behavioral health management, or telephonic nursing experience.
