Care Manager RN (Medicaid/Medicare)

Location: Remote within Florida; applicants must reside in Florida and work Monday-Friday, 8am-5pm Eastern Time

Salary: $56,200-$101,000 per year

What you will do

• Develop, assess, and facilitate complex care management activities for Medicaid and Medicare members with primarily physical health needs.
• Evaluate member needs, barriers to care, and social determinants of health, then create personalized care and service plans.
• Coordinate with members, families, caregivers, providers, specialists, and community resources to address unmet needs.
• Monitor member status, changes in condition, and progress toward care goals, updating plans as needed.
• Provide education on disease processes, care gaps, treatment instructions, care options, referrals, and healthcare benefits.
• Document care management activities and support compliance with state, federal, and payer regulations.

Requirements

• Florida RN license or compact-state RN licensure required.
• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2-4 years of related experience.
• Clinical experience with chronic conditions or diseases, case management, time management, and communication.
• Must reside in Florida and be available during Eastern Time business hours.

• Managed care experience.
• Experience providing telephonic, digital, home, or site-based member outreach.
