Care Manager, Telephonic Nurse

Location: Remote, Oklahoma; must reside in Oklahoma. Occasional travel to the hirify.global Oklahoma office may be required for meetings and training.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ behavioral, physical, environmental, and psychosocial health needs through telephone and face-to-face interactions.
• Create and maintain individualized member care plans focused on optimal wellness and desired outcomes.
• Monitor assessments, evaluations, progress, and barriers to effective care, taking appropriate corrective action.
• Guide members and families toward appropriate healthcare, community, and social service resources.
• Collaborate with providers, community services, and an interdisciplinary care management team to coordinate quality and cost-effective outcomes.
• Determine when face-to-face support is needed and coordinate in-person meetings or visits.

Requirements

• Must reside in Oklahoma.
• Active Registered Nurse license, Licensed Professional Counselor license, or Licensed Clinical Social Worker license.
• At least 1 year of clinical experience and 2 or more years of care management experience.
• Experience coordinating services, care needs, or benefits for Medicaid and/or Medicare enrollees.
• Knowledge of community health, social service agencies, and other community resources.
• Proficiency with Microsoft Word, Excel, and PowerPoint.

Nice to have

• Case management certification.
• Bilingual Spanish-speaking ability.
