Care Manager (BH - Foster Care)

Location: Remote from anywhere within Missouri, United States. Monday–Friday, 8:00 a.m.–5:00 p.m.; telephonic, digital, in-home, and other site visits may be required.

Salary: $56,200–$101,000 per year

What you will do

• Assess complex behavioral health, mental health, substance use disorder, and social determinant needs for adolescents in foster care ages 16–26.
• Develop and monitor personalized care plans for members with high-acuity needs.
• Coordinate care between members, families, caregivers, providers, specialists, community resources, and other care partners.
• Facilitate access to behavioral health services, healthcare, referrals, and community support.
• Educate members and families about treatment options, provider instructions, procedures, referrals, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• Licensed master’s behavioral health professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or an RN according to state contract requirements.
• Behavioral health experience is required.
• Must be located within Missouri for the remote arrangement.
