Community Resource Coordinator II (Healthcare)

Location: Remote from anywhere within Missouri, United States; Monday–Friday, 8am–5pm. Telephonic support with occasional in-person outreach and visits to homes or community organizations.

Salary: $17.84–$28.02 per hour

What you will do

• Connect foster care members with community resources and care services.
• Coordinate outreach resources, educational materials, correspondence, and member support.
• Assist nurses and social workers with outreach, community education, informal guidance, and care coordination.
• Conduct non-clinical health and social needs assessments, including home safety, transportation, employment, and community resource evaluations.
• Conduct telephonic and in-person outreach to locate hard-to-reach individuals and families.
• Document community resources and maintain compliance with health plan standards and policies.

Requirements

• High school diploma or GED.
• 1–2 years of related experience.
• Working knowledge of social determinants of health barriers.
• Ability to provide telephonic support and conduct community outreach.
• Must work remotely from within Missouri.
• Previous experience supporting foster care members is strongly preferred.
