Field Care Manager - RN

Location: Applicants must reside in Cook County, Illinois, within the listed ZIP codes or within a 10-mile radius. Hybrid home-based work requires occasional onsite attendance at the Schaumburg, Illinois market office and 75–90% travel in the Illinois field.

Salary: $78,200–$107,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ physical, environmental, and psychosocial care needs.
• Create care plans and guide members and families toward appropriate healthcare and community resources.
• Identify and resolve barriers that hinder effective care.
• Monitor patient care, progress, and outcomes through ongoing assessments and evaluations.
• Communicate with internal and external stakeholders while following established guidelines and organizational objectives.
• Visit Medicaid members in homes, supportive living facilities, long-term care facilities, and other care settings.

Requirements

• Active Illinois Registered Nurse license without disciplinary action.
• Residence in the specified Cook County ZIP codes or within a 10-mile radius.
• Clinical nursing experience in a hospital, acute care, or home health setting.
• Ability to travel 75–90% within Illinois and attend occasional onsite work in Schaumburg.
• Knowledge of community health, social service agencies, and community resources.
• Experience with electronic medical records and Microsoft Word, Outlook, and Excel, with intermediate to advanced computer skills.

Nice to have

• Bachelor of Science in Nursing.
• Two or more years of in-home case or care management experience.
• Experience with Medicare or Medicaid members, managed care, health promotion, coaching, and wellness.
• Bilingual English and Spanish communication skills.
• Case Management certification or Motivational Interviewing certification and knowledge.
