Registered Nurse Care Manager

Location: Janesville, Wisconsin, United States; remote from home with travel of approximately 40% to member locations in Rock and Jefferson counties. Monday through Friday, 8:00 a.m.–4:30 p.m. CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members’ health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and document member information within required DHS contract timeframes.
• Coordinate care with acute-care and primary-care providers.
• Educate members about disease processes, risks, nutrition, exercise, and lifestyle changes.
• Conduct monthly phone contacts and travel to members’ settings for in-person visits.

Requirements

• Associate degree in nursing.
• Registered Nurse license in the state of Wisconsin in good standing.
• Valid state driver’s license and personal vehicle liability insurance with 100/300/100 limits.
• Ability to work from a dedicated home space that protects member PHI and HIPAA information.
• Ability to meet minimum home internet requirements of 25 Mbps download and 10 Mbps upload.
• Ability to travel approximately 40% for member visits and occasional office training or meetings.

Nice to have

• At least one year of care management experience.
• Previous care management experience.
• Residence within 45 minutes of the assigned coverage area.
