Community Care Partner (AI)

Location: New York, NY; hybrid work with in-person community visits

Salary: $28/hour, plus transportation reimbursement based on the region's appropriate transportation method.

What you will do

• Engage members in person and virtually through phone, text, and email, and enroll them in the care program.
• Conduct scheduled and unscheduled home visits and complete door knocks for hard-to-reach prospective members.
• Identify health and social needs, assess urgency, support advanced care planning, and determine appropriate next steps.
• Connect members with nurses, providers, healthcare services, transportation, financial grants, emotional support, and insurance benefits.
• Facilitate telemedicine visits and coordinate device setup for virtual care.
• Collaborate with nurses, nurse practitioners, social workers, physicians, and other Care Team members while following safety, quality, and escalation procedures.

Requirements

• At least 2 years of experience in a patient-facing role involving care coordination, healthcare navigation, non-clinical case management, resource navigation, or community health work.
• Experience delivering care in members' homes and communities, with the ability to travel freely within the designated geographic area.
• Ability to build trust with members in virtual, telephonic, and in-person settings.
• Strong communication, interpersonal, organization, problem-solving, and record-keeping skills, including handling confidential health information.
• Comfort using technology such as video chat, Google Suite, Slack, and electronic health records, or learning new systems.
• Ability to work in a quiet, secure environment during scheduled remote shifts and travel within the community up to 50–80% of the week at full capacity.

Nice to have

• Community Health Worker certification or formal training.
• Healthcare experience.
• Experience working in a scaling business, fast-paced environment, or startup.
