Palliative Care Social Work Case Manager (Hospice)

What you will do

• Provide telephonic and video social work support to patients with complex needs and life-limiting illness.
• Conduct psychosocial assessments and create individualized care plans addressing clinical, community, socioeconomic, and personal resources.
• Provide emotional, caregiver, grief, and coping support while helping families understand and respond to patients’ needs.
• Lead goals-of-care and advanced-care-planning discussions, including hospice transitions, long-term care planning, and placement coordination.
• Coordinate behavioral health referrals, investigate abuse or neglect concerns, and facilitate family meetings and joint visits.
• Collaborate with clinicians, nurses, care coordinators, local stakeholders, and interdisciplinary teams while documenting clinical interactions in the electronic health record.

Requirements

• Master’s degree in social work and an active license.
• At least 2 years of experience in palliative or hospice care, including goals-of-care conversations, advanced-care planning, grief, loss, and bereavement support.
• Certified hospice and palliative social work credential, or willingness to obtain it within one year of employment.
• Strong interpersonal, communication, problem-solving, and follow-up skills.
• Comfort working with patients by video and phone, using electronic health records and workplace technology.
• Ability to work remotely from the USA.

Nice to have

• Bilingual ability in English and Creole, Spanish, or Vietnamese.
• Experience in health insurance, especially Medicare Advantage.
• Experience working with geriatric patients.
