Social Worker - Primary Care At Home

Location: 1800 N. Main St., Wheaton, Illinois; requires travel to patient homes and work in residences, group homes, and assisted living facilities

Salary: USD 34.53–44.89 per hour

What you will do

• Provide social work services to adult and geriatric homebound patients in residences and through telehealth.
• Conduct psychosocial assessments and develop assessment-based treatment plans for patients and families with complex social, emotional, behavioral, and economic needs.
• Coordinate screening, discharge planning, referrals, community resources, post-acute care, and safe transitions across the continuum of care.
• Provide crisis-oriented psychotherapy, emergency interventions, advocacy, and support for issues including illness adjustment, mental illness, substance abuse, guardianship, abuse, neglect, and bereavement.
• Collaborate with patients, families, care teams, managers, community agencies, and post-acute providers through care conferences and unit rounds.
• Supervise and train lower-level social workers, students, and interns.

Requirements

• At least one year of experience or internship in medical social work in an acute hospital setting or agency.
• Experience with maternal and child populations.
• Licensed Social Worker (LSW).
• Valid driver's license, auto insurance, and a good driving record.
• Ability to travel to patient homes and work in varied home-based environments.

Nice to have

• Two years of experience in an acute hospital setting or at least two years of hospital or community-based social work experience.
• Master's degree in social work.
• Licensed Clinical Social Worker (LCSW).
• ACSW certification.
