Field Care Manager, RN (Medicaid)

Location: Hybrid home-based role for candidates residing in specified southern Illinois counties and ZIP codes, with occasional onsite work at the Schaumburg, Illinois, market office and 75–90% local travel to member homes and care facilities throughout Illinois. Work schedule: Monday–Friday, 8:00 a.m.–5:00 p.m. CST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Identify and resolve barriers that prevent effective care.
• Assess and monitor member care and progress toward desired outcomes.
• Create and update individualized plans of care.
• Visit Medicaid members in homes, supportive living facilities, long-term care facilities, and other care settings.
• Coordinate with internal and external stakeholders and follow established clinical and organizational procedures.

Requirements

• Must reside in the specified Illinois counties and ZIP codes or within a 10-mile radius of the listed ZIP codes.
• Active Illinois Registered Nurse license without disciplinary action.
• Clinical nursing experience in hospital, acute care, or home health settings.
• Ability to travel 75–90% within Illinois and maintain a valid Illinois driver's license and required personal vehicle liability insurance.
• Knowledge of community health, social service agencies, and community resources.
• Ability to use electronic medical records and other healthcare software.

Nice to have

• Bachelor of Science in Nursing.
• Two or more years of in-home case or care management experience.
• Medicare or Medicaid, managed care, health promotion, coaching, or wellness experience.
• Case Management or Motivational Interviewing certification.
• Bilingual English and Spanish communication skills.
