Community Health Worker (Healthcare)

Location: Must reside in Illinois. Hybrid role with up to 75% travel to members' homes, provider organizations, and hirify.global offices in Chicago and Schaumburg; occasional office travel may be required for training or meetings.

Salary: $41,900–$56,600 per year

What you will do

• Conduct in-person assessments of members' healthcare needs, preferences, socioeconomic barriers, and home environments.
• Support case management by addressing social determinants of health and coordinating access to housing, food, income, transportation, and healthcare services.
• Help members schedule physical and behavioral health appointments and improve adherence to care plans, medications, and treatments.
• Provide culturally appropriate health education, coaching, motivational interviewing, and social support.
• Advocate for members and maintain relationships with providers, schools, community resources, and other partner organizations.
• Conduct outreach, health risk screenings, community events, and activities supporting readmission prevention and emergency department diversion.

Requirements

• Must reside in Illinois and be able to travel to members' homes and provider organizations, with up to 75% travel.
• At least two years of experience working with community resources, community health agencies, or social service agencies.
• Valid driver's license, car insurance, and access to an automobile.
• Ability to work 40 hours per week, Monday through Friday, 8:00 AM to 5:00 PM; overtime may be requested.
• Ability to make independent decisions, manage multiple priorities, and work in a fast-paced environment.
• Strong understanding of and respect for cultural and demographic diversity.

Nice to have

• Bachelor's degree in social work or a related health field.
• Community Health Worker training or certification.
• Familiarity with state Medicaid guidelines and experience with Medicaid enrollees.
• Experience supporting members with physical and behavioral health needs and varied health literacy.
• Bilingual communication skills.
