Care Coordinator II (Healthcare)

Location: Field-based role in Batesville, Arkansas, serving Independence, Izard, Sharp, and Fulton Counties. Candidates must reside in Izard, Sharp, or Stone County, Arkansas, or a border city. Travel: 30% required.

Salary: $17.84–$28.02 per hour

What you will do

• Conduct member outreach by phone and through home visits to discuss care plans, services, resources, and next steps.
• Coordinate care with members, caregivers, healthcare providers, community organizations, care managers, and practitioners.
• Respond to member and provider questions or concerns related to care and service plans.
• Perform service assessments and screenings, identify member needs, and make appropriate referrals.
• Document and maintain member records in accordance with state, contractual, and regulatory requirements.
• Provide education about available benefits and resources while supporting continuity of care and member satisfaction.

Requirements

• High school diploma or GED required, with 1–2 years of related experience.
• Alternatively, a bachelor's degree in social science or a health-related field may qualify for the Arkansas Total Care plan.
• For candidates with a high school diploma, at least one year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients is required for the Arkansas Total Care plan.
• Must reside in Arkansas or a border city and be able to travel approximately 30%.
• Valid driver's license required.
• Child and adult maltreatment checks and drug screening are required before hire and on a recurring basis.
