Community Health Worker I (Healthcare)

Location: 541 N. Fairbanks, Chicago, IL, United States; onsite, day shift

Salary: $19.71–$25.62 per hour

What you will do

• Act as a liaison between the health system, social services, and the community.
• Provide telephone support and help patients access resources related to social drivers of health.
• Educate patients and families about social wellness and navigating the healthcare system.
• Collaborate with Population Health, Primary Care, Community Affairs, and federally qualified health centers.
• Complete screenings, provide appropriate referrals, and document patient needs and interventions in the electronic medical record.
• Use motivational interviewing and conflict resolution to resolve issues and system problems.

Requirements

• GED or high school diploma.
• At least two years of customer service experience in a professional office environment.
• Ability to support individuals, families, and communities with outreach, education, advocacy, and resource navigation.
• Ability to comply with regulatory, accreditation, reporting, and documentation requirements.
• Successful completion of a criminal background check is required.

Nice to have

• Community Health Worker Certificate or certificate in progress.
• Community outreach and education experience.
• Patient tracking or contact tracing experience.
• Volunteer experience.
