Field Care Manager Nurse - BSN (Behavioral Health)

Location: Remote in South Carolina; must reside in South Carolina and be located near the Midlands or Upstate area. Field travel of up to 50% is required for face-to-face meetings and collaboration. Schedule: Monday–Friday, 8:00 AM–5:00 PM Eastern Time.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members’ physical, environmental, and psychosocial care needs.
• Guide members and families toward appropriate healthcare, social service, and community resources.
• Develop care plans and monitor progress toward desired health outcomes.
• Identify and resolve barriers affecting effective care delivery.
• Coordinate with staff, providers, members, and families through field visits and face-to-face meetings.

Requirements

• Bachelor’s degree in nursing (BSN) and an active South Carolina nursing license without disciplinary action.
• Residence in the State of South Carolina, preferably near the Midlands or Upstate area.
• At least 2 years of case or care management experience.
• At least 2 years of experience working with behavioral health populations.
• Knowledge of community health, social service agencies, and community resources.
• Ability to use electronic medical records and other healthcare software applications.

Nice to have

• At least 3 years of in-home assessment and care coordination experience.
• Experience with health promotion, coaching, and wellness.
• Managed care experience.
• Bilingual English and Spanish skills.
• Case Management or Motivational Interviewing certification or related knowledge.
