Care Manager (Behavioral Health)

Location: Remote, applicants must reside in Kansas. Work schedules are Tuesday–Saturday or Sunday–Thursday, 10:00 AM–6:00 PM. The role may include phone, digital, in-home, and other site visits.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs related to behavioral health, substance use disorders, social determinants, and available resources.
• Develop and monitor personalized care plans for members with high-acuity and complex needs.
• Coordinate care between members, families, caregivers, providers, specialists, community resources, and multidisciplinary teams.
• Conduct outreach through phone, digital channels, home visits, and other site visits as appropriate.
• Educate members and families about treatment options, referrals, healthcare benefits, provider instructions, and procedures.
• Document care management activities and provide feedback on opportunities to improve quality and cost-effective care.

Requirements

• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• Active Licensed Master’s Behavioral Health Professional license, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN qualification with behavioral health experience according to state contract requirements.
• Strong behavioral health clinical knowledge, assessment, communication, and customer service skills.
• Must reside in Kansas.

Nice to have

• Experience with discharge planning, problem-solving, care coordination, computer systems, organizational management, and time management.
• Experience working with multidisciplinary teams.
