Telephonic Behavioral Health Care Manager

Location: Remote nationwide within the United States; 8-hour shifts scheduled between 8:00 AM and 6:00 PM in the employee's local time zone. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members' behavioral, physical, environmental, and psychosocial health needs in a telephonic setting.
• Guide members and families toward appropriate care, wellbeing, and community resources.
• Use behavioral change, health promotion, and coaching strategies to support member wellness.
• Identify and resolve barriers that prevent effective care.
• Monitor patient progress through ongoing assessments and evaluations and create care plans when needed.
• Make independent care-management decisions while following established guidelines and procedures.

Requirements

• Current valid social service license, such as LMSW, LCSW, LPC, or LMHC.
• Master's degree in a behavioral-health field, including social work, counseling, or a related health discipline, from an accredited university.
• At least 2 years of experience working as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office, shared systems, and troubleshooting general technical issues.
• Dedicated, interruption-free workspace suitable for protecting member PHI and HIPAA information, with internet service providing at least 25 Mbps download and 10 Mbps upload speed.

Nice to have

• Experience supporting patients telephonically.
• Experience in Community Mental Health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.
• Bilingual English and Spanish proficiency; fluency in reading, writing, and speaking will be assessed.
