Telephonic Behavioral Health Care Manager

Location: Remote nationwide within the United States. The role follows an eight-hour shift scheduled between 8:00 AM and 6:00 PM in the local time zone. Occasional travel to hirify.global offices for training or meetings may be required.

Salary: $59,300–$80,900 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess and evaluate members' behavioral, physical, environmental, and psychosocial health needs in a telephonic setting.
• Guide members and families toward appropriate care, wellness, and community resources.
• Develop care plans and monitor progress toward desired health outcomes.
• Identify and resolve barriers that prevent effective care and support behavioral change.
• Use independent judgment to determine appropriate care-management actions while following established procedures.

Requirements

• Current valid social service license, such as LMSW, LCSW, LPC, or LMHC.
• Master's degree in behavioral health, social work, counseling, or a related health discipline from an accredited university.
• At least two years of experience as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office, shared systems, and troubleshooting general technical issues.
• Ability to work remotely from within the United States and maintain a dedicated workspace that protects member PHI and HIPAA information.

Nice to have

• Experience supporting patients by telephone.
• Experience in community mental health, crisis response, Assertive Community Treatment, or substance use disorder treatment.
• Experience working with both children and adults.
• Bilingual English and Spanish proficiency; reading, writing, and speaking fluency will be assessed.
