Care Coordinator (Medicaid Managed Care)

Location: Arlington Heights, Illinois; must reside in Illinois. Remote field position with 75–90% local travel to members' homes, assisted living facilities, long-term care facilities, and other care settings. Schedule: Monday–Friday, 8am–5pm CST, with flexibility and occasional travel to hirify.global offices for training or meetings.

Salary: $53,700–$72,600 per year.

What you will do

• Visit Medicaid members in homes, assisted living facilities, long-term care facilities, and other care settings.
• Assess member needs through face-to-face and telephonic evaluations and develop personalized care plans.
• Plan, implement, coordinate, monitor, and evaluate interventions against members' goals.
• Identify and resolve barriers to care, address immediate gaps, and support progress toward desired outcomes.
• Connect members and families with community resources, referrals, providers, payer sources, and interdisciplinary teams.
• Explain managed care requirements, services, and benefits, and assist with grievances and appeals.

Requirements

• Must reside in Illinois and be able to travel locally 75–90% of the time.
• Bachelor's degree in social sciences, social work, human services, or a related field; or an unrestricted Illinois LPN license, LPC license, or LSW license.
• At least 1 year of healthcare and/or case management experience.
• At least 1 year of experience with Medicare or Medicaid recipients, long-term care, home- and community-based services, or managed care organizations.
• Intermediate to advanced computer skills with Microsoft Word, Excel, Outlook, and electronic medical records or similar systems.
• Valid state driver's license and personal vehicle liability insurance are required.
