Care Coordination, Initiation Team

Location: Remote position for candidates who must reside in Illinois; 75% local field travel is required. Visits may include members' homes, assisted living facilities, long-term care facilities, and other care settings. Schedule: Monday-Friday, 8:00 a.m.-5:00 p.m. CST, with flexibility and occasional travel to hirify.global offices for training or meetings.

Salary: $53,700-$72,600 per year

What you will do

• Conduct comprehensive health risk assessments through telephonic and face-to-face interactions with newly enrolled Medicaid members.
• Assess care needs, identify barriers, and plan interventions that support member well-being in the least restrictive setting.
• Coordinate, monitor, and evaluate case management plans against members' goals and desired outcomes.
• Address immediate gaps in care and access, and connect members and families with healthcare, social, and community resources.
• Provide information about managed care requirements, services, benefits, grievances, and appeals.
• Coordinate with payers, providers, interdisciplinary teams, and other parties involved in member care.

Requirements

• Must reside in Illinois.
• One of the following: bachelor's degree in social sciences, social work, human services, or a related field; unrestricted Illinois LPN license with one year of relevant assessment and service experience; LPC license; or LSW license.
• At least one year of healthcare and/or case management experience.
• At least one year of experience with Medicare or Medicaid recipients, long-term care, home and community-based services, or managed care organizations.
• Intermediate to advanced computer skills with Microsoft Word, Excel, and Outlook, plus experience with electronic medical records and other information systems.
• Valid state driver's license and personal vehicle liability insurance are required. The role is patient-facing and requires TB screening.
