Remote Care Coordinator – Cardiac

Location: 100% remote; Dallas/Fort Worth area preferred

Salary: $26–$32 per hour, based on experience and qualifications

Cardiac Care Alliance is a Management Services Organization building a cardiovascular network through value-based care, proactive care coordination, and evidence-based patient support.

What you will do

• Conduct structured telephone outreach to patients with congestive heart failure and other complex cardiac conditions.
• Manage an assigned patient caseload using risk stratification, assessments, follow-ups, and population health dashboards.
• Review symptoms, medication adherence, functional and psychosocial status, home safety, and social determinants of health.
• Provide patient education and evidence-based self-management support while monitoring for worsening conditions and care gaps.
• Support transitional care within 48 hours after discharge, including medication reconciliation, red-flag screening, and appointment scheduling.
• Document interventions, care plans, and patient goals in accordance with CMS billing standards and coordinate with RNs, cardiologists, PCP offices, and other clinical partners.

Requirements

• Active Medical Assistant certification or an equivalent clinical credential, such as CNA, EMT, or CHW experience.
• At least 2 years of experience in care coordination, case management, or ambulatory care.
• Strong interpersonal communication skills and the ability to build rapport by phone.
• Familiarity with CMS PCM, CCM, and/or TCM requirements and proficiency with care coordination software or EHRs.
• Knowledge of chronic conditions, especially heart failure and related comorbidities.
• Ability to work remotely with a dedicated private workspace, secure internet connection, and compliance with HIPAA and patient privacy policies.

Nice to have

• Familiarity with the Dallas/Fort Worth region.
• Bilingual Spanish and English communication skills.
