Remote Care Coordinator (Cardiac Care)

Location: 100% remote; Dallas/Fort Worth area preferred

Salary: $26–$32 per hour, based on experience and qualifications

Cardiac Care Alliance is a management services organization building a value-based cardiovascular care network with independent cardiologists.

What you will do

• Conduct structured telephonic outreach for patients with congestive heart failure and other complex cardiac conditions.
• Manage an assigned caseload using risk stratification, initial assessments, timely follow-ups, and care-gap monitoring.
• Review symptoms, medication regimens, adherence, functional status, psychosocial needs, home safety, and social determinants of health.
• Provide patient education and support evidence-based self-management, advance care planning, and specialty-care follow-up.
• Complete transitional-care follow-up within 48 hours after discharge, including medication reconciliation, red-flag symptom screening, and appointment scheduling.
• Document interventions, care plans, and patient goals in the care-management platform and communicate with RNs, cardiologists, primary-care offices, and other clinical partners.

Requirements

• Active Medical Assistant certification or an equivalent clinical credential such as CNA, EMT, or CHW experience.
• At least 2 years of experience in care coordination, case management, or ambulatory care.
• Strong communication skills, telephone rapport-building ability, attention to detail, and familiarity with complex care environments.
• Familiarity with CMS Principal Care Management, Chronic Care Management, or Transitional Care Management requirements.
• Technological proficiency with care-coordination software or EHRs and the ability to work independently in a remote environment.
• Dedicated private workspace, secure internet connection, and compliance with HIPAA and patient privacy requirements are required.

Nice to have

• Knowledge of chronic conditions, especially heart failure and associated comorbidities.
• Familiarity with the Dallas/Fort Worth region.
• Bilingual Spanish/English communication skills.
