RN Care Coordinator - Evernorth - Cleveland OH

Location: Based in Cleveland, Ohio; occasional or permanent home work requires cable broadband or fiber internet with at least 10 Mbps download and 5 Mbps upload

Salary: $77,500–$129,100 per year, depending on experience and geographic location; eligible for an annual bonus plan.

What you will do

• Coordinate care for high-risk customer populations and refer individuals to appropriate clinical, health, wellness, and case-management programs.
• Review patient-level data and reports to identify customers for outreach, engagement, and care-gap closure.
• Coordinate communication among employers, primary care providers, onsite health clinics, clinical partners, and Evernorth programs.
• Build relationships with local network providers, guide customers toward high-quality providers, and refer them to onsite health centers and community resources.
• Maintain timely and compliant documentation while supporting clinical, quality, and cost outcomes.
• Communicate with customers and internal and external partners, serving as a resource for non-clinical health care navigation questions.

Requirements

• Registered Nurse with current unrestricted Ohio state licensure and the ability to maintain it.
• At least three years of clinical practice experience.
• Current BLS/CPR/AED certification.
• Strong oral and written communication, interpersonal, problem-solving, computer, and teamwork skills.
• Ability to work independently, communicate with multiple clinical partners, and learn new information and skills quickly.
• Commitment to helping members improve their health and navigate the health care system.

Nice to have

• Multistate nursing licensure.
• Case management, high-risk patient, health coaching, or chronic disease management experience.
• Experience with project development and matrix environments.
• CHES or NBHWC certification.
• Dynamic presentation skills and a demonstrated commitment to healthy behaviors.
