Care Manager (RN)

Location: Remote-KS; certain plan assignments require residence in Illinois or Oregon licensure, and some Florida assignments may require up to 80% local travel

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, access barriers, social determinants of health, and personal care priorities.
• Develop and update personalized care plans and service plans in collaboration with members, caregivers, providers, specialists, and community resources.
• Coordinate timely access to healthcare, referrals, services, and local resources.
• Monitor member status, changes in condition, care gaps, and progress toward goals.
• Educate members and families about disease processes, care options, provider instructions, benefits, and referrals.
• Document care management activities and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• Active RN state licensure and/or compact-state licensure is required.
• YouthCare Illinois assignments require a Bachelor’s degree, Illinois RN licensure, and residence in Illinois.
• Trillium Community Health Plan assignments require Oregon RN licensure.
• Florida Children’s Medical Services assignments require at least two years of pediatric experience and may require up to 80% local travel.
