Location: Onsite Monday–Friday, 8:00 AM–5:00 PM at CarolinaEast Medical Center in New Bern, North Carolina

Salary: $89,174–$142,679 annually

What you will do

• Coordinate care, resources, and services for member populations addressing physical and behavioral health needs.
• Complete comprehensive clinical assessments and develop member-centered treatment, intervention, and discharge plans.
• Perform clinical reviews to assess medical necessity and coordinate services under contractual benefits.
• Collaborate with members, families, providers, physicians, internal partners, and external resources.
• Coordinate equipment, authorizations, referrals, community resources, and funding sources to support safe and effective care transitions.
• Identify barriers to care-plan goals and connect members with appropriate support services.

Requirements

• RN or other applicable clinical licensure required.
• At least 3 years of clinical experience.
• Direct experience and knowledge of case management and utilization management functions and processes.
• Valid US nursing or relevant clinical license required; North Carolina or multistate/compact licensure is required.
• Onsite presence at the assigned facility is required Monday through Friday.
