Care Manager (RN)

Location: Remote in New Jersey with up to 50% local travel for in-home member visits; primary coverage areas include Morris, Sussex, and Warren counties. Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m. ET, with flexibility based on member and business needs.

Salary: $56,200–$101,000 per year

What you will do

• Assess member needs, barriers to care, and social determinants of health, prioritizing member-identified goals.
• Develop and update personalized care plans and service plans in collaboration with members, caregivers, providers, specialists, and community resources.
• Coordinate timely access to healthcare, social, housing, employment, independent living, and other local resources.
• Conduct telephonic, digital, home, and other site outreach, including in-home member visits within the assigned territory.
• Monitor member status and progress, revise interventions, and provide education on conditions, care gaps, referrals, benefits, and provider instructions.
• Document care management activities, support regulatory compliance, and provide feedback on opportunities to improve care quality and cost-effectiveness.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• Active New Jersey Registered Nurse (RN) license or applicable compact state licensure required.
• Ability to conduct up to 50% local travel for in-home member visits.
• Availability for the typical Monday–Friday schedule during ET business hours, with flexibility based on member and business needs.
