Care Manager (RN)

Location: Remote work from New Jersey required; up to 10% statewide travel for field-based member outreach and attendance at team meetings and training in the Iselin office approximately one to three times per year. Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m. EST, with a one-hour lunch.

Salary: $56,200–$101,000 per year

What you will do

• Evaluate member needs, barriers to care, and social determinants of health, then recommend or facilitate appropriate care plans.
• Develop and update personalized care plans and service plans for approximately 180–200 Medicaid and dual-eligible Medicare/Medicaid members.
• Coordinate with members, families, caregivers, providers, specialists, and community resources to address unmet needs.
• Monitor member status, changes in condition, care-plan progress, and care gaps through ongoing follow-up.
• Provide education on disease processes, healthcare benefits, referrals, care options, and provider instructions.
• Document care management activities and support compliance with state, federal, and third-party payer requirements.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2–4 years of related experience.
• Registered Nurse state licensure and/or compact-state licensure required.
• Must reside in New Jersey and be available for statewide travel and occasional Iselin office attendance.
• Ability to work Monday through Friday during EST business hours.
