Care Manager (Behavioral Health)

Location: Remote-OK, field-based; applicants must reside in Oklahoma, Canadian, Cleveland, or Logan County, Oklahoma. The role includes phone, in-home, and other site visits. Schedule: Monday–Friday, 8:00 AM–5:00 PM.

Salary: $56,200–$101,000 per year.

What you will do

• Assess member needs through phone, digital, in-home, and site-visit outreach.
• Develop and monitor personalized care plans for members with high-acuity mental health, substance use disorder, and social determinant needs.
• Coordinate care among members, families, caregivers, healthcare providers, specialists, community resources, and behavioral health services.
• Track treatment changes, complications, clinical symptoms, service access, and member outcomes.
• Educate members and families about treatment options, referrals, healthcare benefits, provider instructions, and available resources.
• Document care management activities and provide feedback on opportunities to improve quality and cost-effectiveness.

Requirements

• Residence in Oklahoma, Canadian, Cleveland, or Logan County, Oklahoma is required.
• Master’s degree in Behavioral Health or Social Work, or a degree from an accredited nursing school.
• Active Licensed Master’s Behavioral Health Professional credential, such as LCSW, LMSW, LMFT, LMHC, or LPC, or an active RN license according to state contract requirements.
• Behavioral health experience and 2–4 years of related experience.
• Computer proficiency and strong communication, customer service, assessment, coordination, and documentation skills.

• Experience working with children, foster care, or the juvenile justice system.
