Care Coordinator II (Foster Care)

Location: Remote from anywhere within the state of Missouri. Monday-Friday, 8am-5pm schedule; member outreach may include home visits.

Salary: $17.84-$28.02 per hour

What you will do

• Conduct telephone outreach and home visits with adolescents in foster care ages 13-26.
• Discuss care and service plans, next steps, resources, concerns, and available benefits.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and care managers.
• Support service assessments and screenings, identify member needs, and make appropriate referrals.
• Document and maintain member records in accordance with state and regulatory requirements.
• Respond to member and provider inquiries while supporting continuity of care and service delivery.

Requirements

• High school diploma or GED.
• 1-2 years of related experience.
• Must work remotely from within Missouri.
• Ability to perform member outreach by phone and through home visits.
• Ability to coordinate with healthcare providers, community partners, members, and caregivers.
