Care Manager (Behavioral Health)

What you will do

• Evaluate member needs through phone, digital, in-home, and other site-based outreach.
• Develop and monitor personalized care plans for members with complex behavioral health, mental health, substance use disorder, and social determinant needs.
• Coordinate care among members, families, providers, specialists, community resources, and care teams.
• Monitor treatment effects, complications, clinical symptoms, service access, and member outcomes.
• Educate members and families about treatment options, referrals, provider instructions, procedures, and healthcare benefits.
• Document care management activities and provide feedback on opportunities to improve care quality and cost effectiveness.

Requirements

• Must reside in Georgia for this remote position.
• Master's degree in Behavioral Health or Social Work, or a degree from an accredited nursing school.
• Active RN license or Licensed Master's Behavioral Health Professional credential required, such as LCSW, LMSW, LMFT, LMHC, or LPC, according to state contract requirements.
• Behavioral health experience and 2–4 years of related experience.
• Strong assessment, time management, communication, customer service, organization, and computer skills.
• Telephonic experience and knowledge of behavioral health care.
