TL;DR
Senior UAS LTSS Assessor (RN) (Long-Term Care/Managed Care): Assessing complex members' medical and support needs and developing personalized long-term care service plans with an accent on clinical assessment, care coordination, and member education. Focus on monitoring changing conditions, coordinating providers and community resources, and conducting field or home visits for New York members.

Location: Remote-NY; Orange or Rockland County residency and New York RN licensure are required. This is a field position that may include home or other site visits once a month or more.

Pay range: $36.21–$65.09 per hour.

What you will do

  • Conduct UAS assessments for complex and high-acuity long-term care members.
  • Develop, monitor, and update personalized care plans and service plans.
  • Coordinate care among members, families, caregivers, physicians, facilities, agencies, specialists, and community providers.
  • Monitor member status, complications, care gaps, progress, waiver eligibility, and potential higher-level care needs.
  • Educate members and caregivers about disease processes, care options, referrals, benefits, and community resources.
  • Document assessments and care management activities in accordance with state, federal, and clinical guidelines.

Requirements

  • Must reside in Orange or Rockland County, New York.
  • Current New York RN licensure or compact state licensure is required.
  • Graduate from an accredited school of nursing or hold a bachelor's degree with 4–6 years of related experience.
  • Bachelor's degree in Nursing is preferred.
  • NP candidates may qualify with current state nurse practitioner licensure.
  • Ability to conduct field and home visits and collaborate with members, caregivers, and healthcare providers.
