RN Care Manager – Cardiac (Cardiac Care): Delivering remote, high-acuity cardiac care management through clinical assessments, triage, individualized care plans, and transitional care programs with an accent on heart failure, medication reconciliation, and patient education. Focus on identifying clinical deterioration, coordinating urgent cardiology care, closing population-health care gaps, and escalating complex needs across the care team.
Location: 100% remote within the United States; Texas or a compact-state RN license required, with preference for candidates based in the Dallas/Fort Worth area.
Salary: $45–$60 per hour, based on experience.

Cardiac Care Alliance is a Management Services Organization building a cardiovascular network and supporting independent cardiologists with value-based care models.
What you will do
- Conduct comprehensive clinical assessments and reassessments for patients with complex cardiac conditions, including heart failure.
- Develop and update individualized, evidence-based care plans and provide patient and caregiver education.
- Perform clinical triage, assess symptom escalation, and determine the need for urgent care or emergency department evaluation.
- Lead transitional care activities after discharge, including medication reconciliation and adherence monitoring.
- Coordinate care with cardiologists, primary care practices, community resources, and other clinical partners.
- Review population health dashboards, address care gaps, document encounters under CMS requirements, and provide clinical oversight to Care Coordinators.
Requirements
- Active RN license in Texas or a compact state.
- At least 2 years of cardiac care or care management experience.
- Knowledge of chronic conditions, especially heart failure and related comorbidities.
- Strong communication and clinical documentation skills.
- Availability for 40 hours per week, Monday through Friday, in a full-time W-2 contract role.
- Dedicated private workspace, secure internet connection, and compliance with HIPAA and patient privacy policies required.
Nice to have
- Experience with electronic health records and Remote Patient Monitoring.
- Bilingual Spanish and English communication skills.
- Residence in the Dallas/Fort Worth area.
