Senior Care Manager (RN)

Location: Applicants must reside in Oklahoma. This is a field-based position with remote, home, telephonic, digital, and site outreach as applicable. Schedule: Monday through Friday, 8:00 AM to 5:00 PM.

Salary: $75,300–$135,400 per year.

What you will do

• Assess member needs and develop, implement, and continuously update personalized care and service plans.
• Coordinate care between members, families, caregivers, healthcare providers, specialists, and community resources.
• Monitor member status, changes in condition, progress toward goals, and barriers requiring escalated interventions.
• Review referrals, intake assessments, and member data to improve care quality and operating performance.
• Educate members and caregivers about disease processes, care options, referrals, provider instructions, benefits, and care gaps.
• Support clinical new hires through precepting, coaching, onboarding, competency checkpoints, and field-practice readiness.

Requirements

• Must reside in Oklahoma.
• Active RN state or compact-state license required.
• Degree from an accredited school of nursing or a bachelor's degree in nursing.
• Four to six years of related experience.
• Experience managing complex medical caseloads, conducting assessments, and communicating with members, families, and providers.
• Experience with children, foster care, or the juvenile justice system is relevant to the role.
