Care Manager (RN) (Healthcare)

Location: Must reside in Kansas, with a preference for the northwest Kansas service area. The role requires approximately 50% travel throughout the assigned Kansas service area, driving across the region and completing in-home member visits; it is not exclusively remote.

Salary: $56,200–$101,000 per year

What you will do

• Enroll members in case management and assess healthcare needs, barriers to care, and social determinants of health.
• Develop and update personalized care plans and service plans with members, families, caregivers, providers, specialists, and community resources.
• Coordinate telephonic, digital, home, and site-based outreach, including regular in-home visits across the assigned Kansas service area.
• Monitor member status, changes in condition, care gaps, and progress toward care plan goals.
• Educate members and caregivers about disease processes, care options, referrals, provider instructions, and healthcare benefits.
• Document care management activities and member information in compliance with state, federal, and payer requirements, while identifying opportunities to improve care quality and cost effectiveness.

Requirements

• Must reside in Kansas and be comfortable with approximately 50% regional travel and in-home member visits.
• Degree from an accredited school of nursing or a bachelor's degree in nursing, plus 2–4 years of related experience.
• Current RN state licensure and/or compact-state licensure required.
• Strong communication, organization, care coordination, assessment, documentation, and member education skills.
• For specific plans, additional requirements may apply: Illinois RN licensure and residence in Illinois, Oregon RN licensure, or pediatric experience for Florida Children's Medical Services.
