Senior LTSS Service Care Manager (RN)

Location: Remote-NY; residency in Schuyler, Steuben, Livingston, or Allegany County is required. This is a field position that may include home or other site visits once a month or more.

Salary: $36.21–$65.09 per hour

What you will do

• Conduct UAS assessments for the Fidelis MLTC member population.
• Evaluate complex and high-acuity members' medical, long-term care, and support service needs.
• Develop, monitor, and update personalized care plans and service plans.
• Coordinate providers, specialists, caregivers, community resources, referrals, treatments, and service authorizations.
• Educate members and families on care options, disease processes, benefits, referrals, and available community resources.
• Document care management activities, monitor outcomes and status changes, and support quality improvement and regulatory compliance.

Requirements

• Residency in Schuyler, Steuben, Livingston, or Allegany County, New York, is required.
• Current New York RN licensure is required; a current state nurse practitioner license may also qualify.
• Graduate from an accredited school of nursing or hold a bachelor's degree with 4–6 years of related experience.
• A bachelor's degree in nursing is preferred.
• Ability to conduct field assessments and occasional home or site visits.
