Care Manager (RN) - OB

Location: Remote from anywhere within the state of Missouri. Monday-Friday, 8am-5pm.

Salary: $56,200-$101,000 per year.

What you will do

• Assess pregnant members’ physical care needs, barriers to care, social determinants of health, and personal priorities.
• Develop and update personalized care plans and service plans in collaboration with members, families, caregivers, providers, specialists, and community resources.
• Coordinate timely access to healthcare services, referrals, providers, and local support resources.
• Provide education on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Conduct telephonic, digital, home, or other site outreach, and monitor member status and progress toward care goals.
• Document care management activities and member information in compliance with state, federal, and payer requirements, while identifying opportunities to improve care quality and cost effectiveness.

Requirements

• Degree from an accredited school of nursing or a Bachelor’s degree in Nursing.
• 2-4 years of related experience.
• Active RN state licensure and/or compact state licensure required.
• Must be able to work remotely from within Missouri.
